Composite Bonding Model Applications

We’re looking for patients who are wanting to go ahead with a full mouth of Composite Veneers and who are available during the week for a 30 minute consultation at our London Harley Street Clinic. All applicants must be available on these days, anyone who isn’t available will not be considered. All applicants must be ready to go ahead with the treatment as we will be looking to confirm and book treatment on the day of the consultation, if you’re successful. A £50 deposit will be taken on booking which will be deducted from the cost of treatment or is refundable if for any reason you’re unsuitable for modelling treatment.

We’re offering this treatment for £150 per tooth! If this is something you’d be interested in, please ensure you meet the below requirements and complete the application form to be considered.

 Requirements include:

• Must have straight teeth
• Must be over 18 years old
• Must consent to filming & photography throughout treatment
• Hygiene prior to treatment (charged separately)
• OPG X-ray prior to treatment (charged separately)

1. Only available to new patients
2. Must be available to attend consultations on the dates specified
3. Must consent to filming and photography
4. Must provide an honest review and testimonial via Google, Trustpilot and recorded video
5. Must be happy to tag us and be tagged in content on Instagram
6. Must be over 18 years old
7. Must compete application form to be considered for a consultation
8. Must understand that a hygiene appointment, x-ray and check up are necessary before any treatment can go ahead
9. Whitening, night guard, hygiene & x-ray costs are not included
10. £150 a tooth is for composite veneers only
11. Must pay £50 deposit on booking to secure consultation appointment and understand this is deducted from the cost of treatment. This is not refundable if patient decides against going ahead with the treatment.
