How to join the Plan

You can choose to sign up directly at the Practice, or you can request a staff member to send you a link via email/text.

Your registration

You may withdraw from the Pro Dental Plan by giving three months’ notice in writingto CODEplan. If you leave the plan within 12 months of receiving discounted dental treatment the amount of the discount received will be payable to the Practice immediately. It is the patient’s responsibility to book appointments. If you fail to book in or attend your routine appointment on time, the practice will be unable to back date or refund for the missed appointment. If you change address, please notify CODEplan and the practice. If you change your bank details, notify CODEplan.
